I was a bit skeptical about this wedge, but I really needed some relief from my acid reflux, and my neck was sore from trying to use regular pillows to prop myself up. I could not be happier with this product. It is very comfortable, and works like a charm. No more relux problems at night, and my neck pain is gone. I am a back and side sleeper, and find it comfortable in both positions. I don't experience the slipping that other reviewers have. I also don't feel the wedge is too narrow for me, but I do see how someone with a large frame might have some arm discomfort when sleeping on their back due to their arms not fitting onto the wedge. This could probably be alleviated with a thick pillow on each side to support their arms. I would recommend purchasing a second cover so they can be switched out and laundered more often.
