Your problem is very common but hard to treat. Firstly, you must wear a sunscreen every day, all year round. It must contain UVA and UVB radiation protection. An SPF of 20 with UVA/B is adequate.
Secondly, the skin can be lightened with different products over the counter, or with procedures available at a medical clinic specializing in skin care. The treatments could involve the use of high grade vitamin C at 10%, medical skin peels or microdermabrasion.
Good luck .

After doing much research and numerous consultations with derms and esthiticians, I was told IPL is good for the vascular componants of poikiloderma (persistent redness, broken blood vessels, etc)
It will not improve texture though, which was my main issue (chicken skin texture - bumps all over my neck and upper chest) When they try to sell you the package deals, they tell you it will improve texture but that is only a temporary improvement because your skin is swollen (plumped up) from the IPL treatments, thus giving the illusion it has resolved the texture.
I had some spot treatments done w/ a low setting of IPL to get rid of individual spots (cherry angiomas and brown spots) but I would not risk doing my whole neck and chest due to potential for a less than desirable outcome and possible scarring.
I am currently using Retin A for the textural issue (just started in May) and I hoping that will help.
Unfortunately, poikiloderma proves to be very hard to treat, especially the texture and atrophy and one has to understand there will be limitations on what can be done.
It can be a stressful and disfiguring skin condition for those who suffer from it.
I ought to know.

What about IPL or Retin A for sun damage on the neck? Which is better?? What about poikiloderma, what is the best treatment?
