After several revision rhinoplasties many areas of my nose seem to have puffiness, no flat to the skin. Is there a way to fix this?

I don't know if this puffiness is from scar tissue, or the surgeon being over aggressive on my thick skin, or pollybeak deformity, or something else. A gortex implant was recently cut out on the bridge after 3 months of it being infected and replaced with a thick Alloderm at the same time. The surgeon said he had to cut it from the outside oppose to removing from the inside because it had been in there for 2 years and it may have ripped my skin. I noticed a lot of puffiness after this surgery. Many areas of the skin are not smooth or lay flat. The puffiness is not on the bridge where the surgery was, but elsewhere on the nose.
An older surgery left me with a very large puffy look between my nostril and bridge. This one is hard and I believe it is probably scar tissue. I would be grateful for any thoughts on this. It seems inevitable I will never have a decent nose after trying so hard for 25 years.
