transnasal wiring these days is a procedure to salvage a poor result where the nasal bones have BOTH been separated from their periosteal soft tissue anchoring, the attachment to the cheek bones were divided as were the attachments to the underlying Upper lateral cartilages. It is usually done intraoperatively when the nose collapses and NOTHING but placement of these supporting transnasal wires will hold them long enough to allow tissue adherence and bone healing in the proper position.
